HomeMy WebLinkAboutFMCoC HEAP Final ReportHEAP Final Report - Expenditures Introduction In the following form, complete your jurisdiction's final HEAP Expenditure Report. All data should be cumulative (i.e. from the grant start date through June 30, 2021). Submit the completed report by October 15, 2021. The report will include questions about: 1. Jurisdiction Selection: Identify your jurisdiction and provide contact information 2. Reported Final Totals: Total funds expended and total interest accrued. 3. Spending by Category: A final categorical break down of expenditures (administrative, capital improvements, services, rental assistance/subsidies, other) on non-youth activities. 4. Youth Set-Aside Detail: A final categorical break down of Youth Set-Aside expended. 5. Interest Expended Detail: A final categorical break down of interest accrued expended. 6. Comments and Certification For resources to assist in completing and submitting this report, refer to the HEAP Final Report Overview. Name First Last Phone Email Reported Final Expenditures In the following section, we want to capture the total final expenditures for the HEAP grant. Reminder: this should be cumulative (i.e. from the grant start date through June 30, 2021). When calculating these totals, be sure to include all spending (administrative, youth set-aside, capital improvements, services, etc.). Total HEAP Funds Expended Total Interest Accrued from HEAP Funds If your jurisdiction held HEAP funds in an interest bearing account, enter the total interest accrued on HEAP funds. If your jurisdiction did not hold funds in an interest bearing account, enter "0.00". Total Interest Accrued Expended State how much of the accrued interest has been expended. Section 5 will ask for a categorical breakdown of interest spending. Spending By Category In the following section, report your jurisdiction's final categorical expenditures. Reminder: this should be cumulative (i.e. from the grant start date through June 30, 2021). Enter "0.00" if you did not spend any funds in a given category. The sum of all categories should equal the amount entered for your jurisdiction's HEAP funds expended. In the prior section you indicated this amount was Total Administrative This should total no more than 5% of your jurisdiction's total allocation. Do not include interest accrued expended. Details in this section are particularly important when: (1) a project does not capture client service or outcome data recorded in HMIS AND/OR (2) the project made an impact that is not necessarily related to or captured in client service and outcome data recorded in HMIS. Projects not typically captured in HMIS include: capital projects, projects funding domestic violence service providers, hygiene projects, and some pilot projects. Impacts that are not easily captured in client service and outcome data could include: expanding a service's geographic reach, strategic planning and/or leveraging funds, capacity building, and strengthening partnerships.Scenario 1: A jurisdiction funded a capital project(acquiring a 50 room motel through project homekey) that braided $1million in other state dollars with $500 thousand in local funds and $500 thousand in HEAP funds. In this case, the grantee might highlight qualitative impacts that help contextualize or add to client service data. In this case the grantee might highlight what impact adding shower facilities has on the wider program/service capabilities. Scenario 4: A jurisdiction renovated an existing emergency shelter to create more space and privacy for clients. This renovation permanently reduced the number of people a facility can serve at any given time. In this case, the grantee might highlight what the renovation did and/or how the renovation supports clients.
