THRESHOLD FACTORS Name Description Status Policies Remain Complaint All of the renewal project’s relevant policies and procedures were examined as part of a previous year’s review and rank process. The project should pass this threshold factor if it certifies that none of its policies have changed in ways that could impair its federal obligations, including: • identifying and lowering its barriers to housing in line with a Housing First approach • participating in coordinated entry to the extent possible for this project type • entering data for all CoC-funded beds into HMIS (or parallel database for domestic violence services). • maintaining adequate internal financial controls, record maintenance and management, and policies regarding termination of assistance, client appeals, ADA and fair housing requirements, and confidentiality. • providing equal access and fair housing without regard to sexual orientation, gender identity, local residency status, or any other protected category • only accepting new participants if they can be documented as eligible for this project’s program type based on their housing and disability status Pass / Fail Consumer Input The agency engages homeless and formerly homeless clients in program design and policy making by including them on its board of directors or staff, by having a consumer advisory board that meets regularly, by administering consumer satisfaction surveys, and/or by convening client focus groups. The agency must specifically indicate which of these option(s) it is using to gather consumer input. Pass / Must Fix FMCoC Membership The agency is a member in good standing of the FMCoC. Pass / Must Fix Match The project demonstrates 25% match per grant using match letters that specify the kind and amount of resources to be used or donated. These letters do not have to be signed. COMPLIANCE (20 pts.) Name SPMs Description Score Audit Findings Projects were instructed to submit a copy of their most recent audit report or monitoring report. Any type of report can be used – the audit could be from HUD, from a direct recipient, from an accountant, etc. The report should be dated no earlier than 1/1/2018. Award the full 8 points if • The project attaches a recent report with no significant negative findings. Award up to 5 points if: • The project did not attach a recent report, but the project convincingly explains why it was not monitored or audited since 1/1/2018. • The project attached a report with negative findings, but the project convincingly explains how it has corrected the negative findings. Otherwise, award 0 points. Up to 8 points Coordinated Entry 1.1 1.2 Award 2 points if the project reported at least 80% of its bed openings to the Coordinated Entry System before filling those beds. Also, award 2 points if the project accepted at least 80% of the referrals it received from the Coordinated Entry System, or if the project received no referrals. Finally, award up to 3 additional points based on the project’s subjective description of how it contributes to the Coordinated Entry System, including but not limited to participating in CE workgroups, participating in case conferencing, and serving as an official access site. HOUSING PERFORMANCE (24 pts.) Name SPM Description Score Permanent Supportive Housing (PSH) Housing Retention 7b.2 Count each person who either remained in the project at the end of the measurement period or exited to permanent housing. Divide this count by the total number of people who participated in the project during the measurement period, excluding people who passed away or who exited to foster care, nursing homes, or non-psychiatric hospitals or inpatient medical facilities, and then apply the scale to the right. ≥ 95% = 24 90% - 94.9% = 18 85% - 89.9% = 12 80% - 84.9% = 6 < 80% = 0 Rapid Re-Housing and/or Transitional Housing (RRH/TH) Efficient Housing 7b.1 This factor measures how efficiently the program is able to move clients from homelessness into permanent housing. Count the number of people who exited from the program into permanent housing, and divide by the size of the program, as measured by the number of beds that the program said it would provide in its e-SNAPs application. Then apply the scale to the right. ≥ 0.8 PH exits per bed = 6 points 0.5 – 0.79 PH exits per bed = 4 points 0.3 – 0.49 PH exits per bed = 2 points < 0.3 PH exits per bed = 0 points Reliable Housing 7b.1 This factor measures how reliably the program is able to move clients from homelessness into permanent housing. Count the number of people who exited from the program into permanent housing, and divide by the total number of people who left the program. Exclude people who passed away or who exited to foster care, nursing homes, or non-psychiatric hospitals or inpatient medical facilities; do not count these people in the numerator or denominator. If no clients left the program during the competition period, award full credit for this scoring factor. Otherwise, apply the scale to the right. ≥ 85% = 18 80% - 84.9% = 12 75% - 79.9% = 6 < 70% = 0 RTH The FY2021 competition may also measure RRH programs’ returns to homelessness. These are the successes. Divide the number of successes by the number of living participants (minus the number of stayers not yet due for an annual assessment) and apply the scale to the right. ≥ 95% = 3 90% - 94.9% = 2 80% - 89.9% = 1 < 80% = 0 Employment and Volunteerism 4.1 4.4 Award points for the project’s narrative based on the quality of the program’s efforts to connect clients to employment and volunteer opportunities. Up to 2 points Improve Safety for DV Survivors Award points based on the extent to which the project will improve safety for survivors of domestic violence, dating violence, sexual assault, stalking, and/or trafficking. Up to 3 points Quality of Services 4.1 4.2 4.3 4.4 4.5 4.6 Award points based on the project’s narrative based on the quality of the program’s supportive services. FULL UTILIZATION (24 pts.) Name SPM Description Score Bed Utilization 3.1 3.2 Count the average number of people enrolled in the project on the last Wednesday of each quarter, and divide it by the number of beds promised in e-snaps to get the bed utilization rate. Then, award points based on the scale on the right. You may add up to 2 additional points (to a maximum of 12) based on the project’s narrative if the project: • Faced circumstances beyond its control that made it difficult or impossible to fully utilize grant resources (this could include a consolidation or expansion with a project that has less than a year of operating data), and • has a concrete, plausible plan to improve utilization of grant resources for future years ≥ 90% = 12 80% - 89.9% = 10 70% - 79.9% = 8 60% - 69.9% = 6 50% - 59.9% = 4 40% - 49.9% = 2 < 40% = 0 Grant Spenddown 3.1 3.2 Divide the amount of money drawn down from e-LOCCs during the project’s most recently completed contract, by the amount of CoC funding shown for that project on the corresponding GIW. Then, award points based on the scale on the right. You may add up to 2 additional points (to a maximum of 12) based on the project’s narrative if the project: • Faced circumstances beyond its control that made it difficult or impossible to fully utilize grant resources (this could include a consolidation or expansion with a project that has less than a year of operating data), and • has a concrete, plausible plan to improve grant spenddown in future years ≥ 90% = 12 80% - 89.9% = 10 70% - 79.9% = 8 60% - 69.9% = 6 50% - 59.9% = 4 40% - 49.9% = 2 < 40% = 0 FRESNO-MADERA CONTINUUM OF CARE 2020 DRAFT RENEWAL PROJECT SCORING TOOL (SIMPLIFIED) Approved by the Fresno-Madera Continuum of Care on xxx, 2020. 1 6. PRIORITIZATION (15 pts.) Name SPM Description Score Housing First 2 Award up to 5 points based on the project’s narrative if the narrative shows that the project: • Will check all “Housing First” boxes in e-snaps • Admits new clients without barriers based on income, sobriety, criminal records, or mental health • Does not require clients to participate in mandatory classes, therapy, job training, or interventions. • Does not impose restrictions on clients that go beyond what is typically covered in an ordinary lease agreement • Is able to address unpleasant client behavior without exiting or evicting those clients The Panel has discretion to adjust this score up or down based on an optional interview of the program and/or an optional review of the project’s policies and procedures. Up to 5 points Special Populations 1.1 1.2 Award up to 4 points if the project targets one or more of the following specialized populations: (a) Homeless Youth, (b) Domestic Violence survivors, (c) Homeless Families with Children, (d) Chronic Homeless, and/or (e) Homeless Veterans In order to earn credit for this factor based on serving the Chronically Homeless population, a PSH project must check the box for DedicatedPLUS or 100% Dedicated in e-SNAPs. Up to 4 points Severity of Needs 1.1 1.2 Award up to 4 points based on the project’s plan to meet specific needs and vulnerabilities, including: low or no income, current or past substance use, a history of victimization such as domestic violence or sexual assault, criminal histories, and/or chronic homelessness. Up to 4 points Fair Housing 1.1 1.2 Award up to 2 points if the project affirmatively markets housing and supportive services to eligible persons regardless of race, color, national origin, limited English proficiency, religion, sex, gender identity, sexual orientation, age, familial status, and/or disability. This means going beyond mere non-discrimination and taking positive steps to welcome those who would otherwise be least likely to apply. Up to 2 points
