Measure 7b.2 Exits to Permanent Housing Destinations or retention of Permanent Housing –this measures successful housing outcomes for people served in permanent housing projects other than rapid rehousing. Step 1: Introductions - Who is represented in this workgroup? (5 minutes) Step 2: Describe – General observation from the graph Metric 7b.1 and 7b.2, specifically looking at the changes from year to year and how we compare against the National Average. • We rock! We’ve been consistent. Great at housing but not at keeping them housed • Above national average for 7b1 • Just below of 7b2 • Fairly consistent throughout the years Step 3: Analyze – (10 minutes) 1. Is the data accurately reflecting what is happening across the system (i.e., are all permanent housing projects and RRH projects reflected in the data and, if not, what can be done to engage them)? • No, not every program enters into HMIS. Reporting is always changing. We want to know where they are exiting, was it a positive exit. • Cannot know based on the graph 2. What best-practices or services are you providing in your project to help with a positive exit or retention (aka, why is your project successful)? • Can’t measure success off the graph not enough information • Life skills • Connecting to services (medical, etc) • Healthy relationships • Conflict management • Financial management • Substance abuse services Step 4: Improvement Goals (10 minutes) 1. Should exit and retention data be shared to determine whether one population or subpopulation (i.e., individuals, families, chronic, etc.) has a relatively higher rate of successful exits, if so how often should it be shared with the CoC? • Yes, we can learn from other subpopulations. Report bi-monthly/ quarterly 2. Recommendations for short term and long term goals to improve performance? • Immediate: extended (after they left) case management, strengthen the front end to engage. • Cultural/change – motivation (change of minds) • Continuity, more team building gatherings, breakdown of the information given 3. What additional training for staff is needed to increase exits to permanent housing (i.e. Housing First, Promoting long-term stability, tips for working with landlords, motivation interview, linking to mainstream services/employment. etc.)? Please prioritize need of training based on group consensus. • Entering accurate data into HMIS • Extended case management • Training outside of housing specific issues, ex. Parenting • Housing First • Motivational interviewing • Linking to mainstream services • Promoting long-term stability • Tips for working with landlords.
