Step 1: Introductions - Who is represented in this workgroup? (5 minutes) • Westcare • EOC • Poverello House • Kings View Step 2: Describe – General observation from the graph Metric 7a.1 % Successful SO Exits and the chart below (5 minutes) In order to remain comparable to 57% in FY 2017, there should have been 373 successful exits; 138 to temporary and some institutional destinations; 235 to permanent housing destinations. • Would like to have the destinations listed out • In 2017 data collected increased and has continued to increase • In 2016 the database was changed, Homelink was phased out and transitioned to HMIS • There have been several By Name List clean-ups that have assisted in reporting more accurate date • Outreach teams are reaching more people and expanding to rural communities • Housing availability is drastically affecting the percentage of successful exits • There is a chunk of data missing from MAP since they do not have access to HMIS. Is the data accurately reflecting what is happening across the system? • It is starting to, but it can still be improved 2. What key factors do you see in your projects causing the decrease in the percent of positive exits (i.e., number of outreach staff, reasons people are becoming homeless, availability in temporary, institutional and permanent housing resources)? • Outreach staff increased and more data is being captured. • Built for Zero team focused on top 20 of most vulnerable individuals • Staff turnover and training contributed to the percent of positive exits Step 4: Improvement Goals (10 minutes) 1. What data, information, is missing or needed to develop goals on improving this performance? • What linkages and services were provided to the client during street outreach 2. Recommendations for short term and long term goals to improve performance (i.e. review reports more frequently, additional training on street outreach engagement)? • Better outcome reporting, more detailed concrete/tangible information 3. How often would you like this data to be reviewed at CoC meetings? • Monthly
