HomeMy WebLinkAboutSystem Planning Workgroups M1MEASURE 1 – Length of Time Person Remains Homeless Definitions; • Median – set of numbers where half the numbers are lower and half the numbers are higher. • Average – set of numbers where the total of those numbers are divided by the number of items in that set. • When the average is higher than the median, that tells you that the length of time homeless is significantly more common than lower lengths of time homeless. Step 1: Introductions - Who is represented in this workgroup? Of Schools • Marjoree Mason Center • FEOC Transitional Youth Shelter Step 2: Describe – General observation from the graph, initial thoughts? (5 minutes) • National average is stable, Fresno is improving/longer days in shelter • Not clear if staying on streets or going to perm housing • After transitional housing –how much time does it take to be accepted into programs or housing while in transitional housing so that they may be why staying longer. Step 3: Analyze – (10 minutes) 1. Is the data accurately reflecting what is happening across the system (i.e. are all projects being accounted for in this measure, what projects are missing, and how can they be engaged)? • Differentiate reasons for leaving housing or exiting to streets or permanent housing • Would like to look at the demographic data specifically youth that are leaving • Are exemptions being documented • Transitional housing shelters/agencies being left out • Youth shelters missing (need more of) 2. What key factor do you see in your projects causing the reduction in the average and median time homeless (i.e. best practices)? • Intensive/good case management- matching to best programs/housing for success, contact with other programs/navigators. • Cohesive communication • Diversion of helping place clients – could be simple solutions • Meet w/landlords – assistance for rent deposits • Landlord engagement • Permanent housing Step 4: Improvement Goals (10 minutes) 1. What additional data, or information, is missing/needed to develop goals on improving this performance? • Better data entry from all agencies – consistent reporting so we can find if diversion works within 1 day, what services are available. • How many individuals are being double counted by leaving ES, SH and going to other agencies – inflating shelter numbers? 2. Recommendations for short term and long term goals? • SHORT TERM – real time numbers • Invites to City Members – council, staffers, interns to be involved in these planning meetings- help prevent City from working against all agencies involved 3. How often would you like this data to be reviewed at CoC meetings? • Quarterly – short term • Yearly – long term Additional input from HMIS Committee – • All projects begin to utilize case manager tab in HMIS, in order to have a point of contact • Utilize clients notes more often so information can be shared • Projects are still siloed- communication needs to be increased and take away the “my client” mentality • There is an improvement in the data quality for ES projects, but the importance of data quality needs to be stressed from the top and dispersed to staff • Let staff know what responsibilities they have through the coordinated process of getting a person housed. i.e., responsibility of Navigator when the person is accepted to RRH or PSH, does the RRH/PSH project take over in finding a unit? Should there be shared duties between Navigator and RRH/PSH case managers. What guidance has been given about this. • Determine how to utilize the data to determine who the attention should be given to. • Bring leadership back to table for guidance and accountability, similar to 25 Cities.
